Membership to the SNFFC, Inc. is limited to current Federal Employees and Contractors, and membership forms must be completed AND submitted EACH Federal Fiscal Year.  Member fees are based on site location and employment status, and may be covered via check, money order, or credit card. Application

 

To become and remain an SNFFC, Inc. member in good standing, you MUST complete an annual Membership Commitment form EVERY year.  This form, along with your signed Waiver and membership fees MUST be received by a member of our Fitness Team before beginning your first workout.

Payroll Deduction Instructions

 

If desired, membership dues may be paid using payroll deductions through MyPay or Employee Express.  You will need to go into your personal account to set up the allotment, and instructions can be found here.

Waiver

 

All SNFFC, Inc. members are required to complete a Waiver of Liability prior to working out in the fitness center.  This Waiver is your statement of your health and allows us to get you on the fitness floor without further medical documentation.

Rules of Conduct

 

All SNFFC, Inc. members are required to read and sign the SNAFC, Inc. Rules of Conduct prior to working out in the fitness center.  All members are expected to conduct themselves with decorum and to observe proper gym etiquette for the comfort and protection of themselves and other members and guests.

Locker Rental Form

 

This form is required for any member wishing to rent a personal locker. Rental periods extend for the duration of the fiscal year, and must be renewed annually. Locker cost varies, depending on the size and location of the locker. Please speak with a Fitness Team Member to determine the best fit for your needs.
